Our Team

Registered Nurses/LVNs

Provide direct patient care, coordinate the total home health care program, train and educate the family or designated caregivers to continue the plan of care until professional services are no longer needed, educate patients: diabetic teaching, bowel and bladder training, disease management, pain management, wound and ostomy care, and the proper use of medications.

Certified Home Health Aides

Provide personal care for the patient and assist them with the daily activities during recovery. They are under the supervision of the RN as ordered by the Physician.

Physical Therapist

Assist in regaining mobility and independence through strengthening and endurance. Instructions in transfer techniques, propr use walking aids, prosthetic, and other assistive devices, and instructions on performing daily activities and grooming patients safely at home.

Occupational Therapist

Involve the patient in developing, improving, or restoring functions of a daily living affected by illness or injury. Perptual motor abilities, sensory impairment, saftey in home environments and difficulties with coordination, balance, and walking of muscle dysfunction.

Speech Language Pathologist

Assist the paitent in improving communication skllls for skills that may not have fully recovered. They also treat swallowing difficulties.

Medical Social Workers

Provide assistance in obtaining community resources, short-term counseling and long-term planning for health care needs. Assessment of patients psychological needs, conseling of family and friends related to the patient, assessment of patients financial resources, and guidance in the use of appropriate community resources
