What is home healthcare?
Home healthcare is a service that provides professional health care to patients in the comfort and privacy of their own homes for conditions that have required or have led to previous hospitalization.

If you have had a recent hospitalization. Minimize hospitalizations and emergency room visits. Education and disease management.
Conditions for which home health care is beneficial.
Uncontrolled Hypertension, diabetes, and those who are too weak for ADLs.
Who can refer patients for home healthcare?
Physicians, the patient, or family.
Who qualifies for home healthcare?
Generally, any patient requiring skilled medical intervention related to acute/chronic disease exacerbation, injury, or illness and is experiencing a normal inability to leave the home for treatment due to physical or psychological limitations, qualifies for home care services. Qualifications may vary per insurance provider, so it is always best to check with your benefits department to ensure eligibility. Medicare/Medicaid clients must be home-bound to be eligible. Patients must also be under the care of a physician (i.e., medical doctor (MD), osteopath (DO), podiatrist (PD), and/or psychiatrist). We accept referrals from patients themselves, physicians, case managers, community agencies, relatives, friends and neighbors, hospitals, and welfare agencies.
How long can the nurses and therapists come to see me?
As long as a physician orders home care services that he/she deems medically necessary.
